Read following instructions carefully before filling the form.

Instructions:  

1. Membership 

   1. General Member:       Rs. 1200/- per annum

   2. Youth Wing:             Rs. 100/- per annum

   3. Volunteer Wing:        Rs. 100/- per annum

   4. Women Wing:           Rs. 100/- per annum

2.  Please hand over Membership Form duly filled and signed to any of the Executive Body member or Active member.
   OR Send by mail at our following Office address:
   Labbaik for Welfare, 11-B Fareed Plaza, 65-Shadman Market, Lahore-54610.

3. Form must carry the following documents to complete the membership requirement:

   1. Copy of National ID Card.

   2. One latest passport size photograph.

   3. Cross Cheque / Pay Order / Bank Draft in the name of “Labbaik for Welfare”

4. Remember to get official receipt for payment from the authorized recipient. 

You can also pay Donations / Zakat / Charity to Labbaik by Cross Cheque / Pay Order / Bank Draft in the name of “Labbaik for Welfare”.

It is very important to note that all donations, Zakat, Charity, Membership Fee and other help should be paid to Labbaik for Welfare by Cross Cheque / Pay Order / Demand Draft in the name of “Labbaik for Welfare” or by Cash only to the authorized persons(s) i.e. Executive Body members, District members and Office Secretary only.
